- Social Security Card
- Professional License or Certified Nursing Assistant Certification
- CPR Card (only applies to LPN and RN applicants)
- Proof of current PPD (if you cannot provide a current copy of a PPD you can have this done). You must have Proof of a current 2 step PPD.
- Proof of current physical
- Proof of Mandatory In-Services (only if you have attended within last year)
- Completed application or resume with 2 references listed
- Criminal background check will be done by PRN if you do not have a recent one
- All employees have a pre-employment drug screening
- Face to face interview once all paperwork is collected to determine if applicant will be hired
