We are looking for compassionate care providers with a desire for professional development and a commitment to excellence. We offer flexible scheduling options as well. If this sounds like you, and you meet the criteria, complete the online application to be an employee or simply join our FCH Registry as an independent contractor that can become a part of the on-call pool.

The states in which we operate have the following minimum employment requirements — in some cases, additional requirements may apply:

  • Current certification in the state of practice
  • Current CPR Certification
  • Current physical and PPD or Chest X-ray
  • Employment Eligibility Documents
  • Reliable Transportation
  • Must pass background check and drug screening
  • Passion for caring for elders
  • Kindness and confidence
  • Strong work ethic and dedication

We require the following documentation prior to hiring and orientation based on the location to which you wish to apply:

  2. Current (less than a year old) Employee Physical Form completed by United States Doctor (Physical form includes the 1st & 2nd Step PPD or Chest X ray) Download PDF

  3. Immigration Documents ( U.S passport or Permanent Resident Card, State ID or Driver’s License, Social Security Card)

  4. 2×2 passport sized photo on white background

  5. Successful completion of Allied Care competency exam (80% or higher)

Please download and read the pre-exam handout and then complete the exam online. If you pass the exam and your documentation meets Agency requirements, we may then contact you to schedule an interview.
